The Opportunity
We are seeking an exceptional Advanced Practice Provider (APN or PA) to serve as a clinical partner and trusted player on the SHIFT care team. You will serve as a point of leverage and support to our CEO (Dr. Ari Levy) and CMO (Dr. Brian Hollett), ensuring the highest level of care for their panels. You will work closely with the CEO (Dr. Ari Levy), the Chief Medical Officer (Dr. Brian Hollett), and the rest of the care team to ensure seamless, responsive care for the members of Dr. Levy and Dr. Hollett. You will manage daily clinical workflows, respond promptly to clinical needs, and coordinate care to enable SHIFT’s CEO and CMO to maintain exceptional service levels even during periods of non-clinical commitments.
You will serve as a trusted clinical partner, providing timely support, triage, and coordination services that enhance the overall patient experience and help sustain the gold standard of care.
Who You Are
- Licensed Advanced Practice Nurse (APN) or Physician Assistant (PA) in Illinois
- 3–7 years of clinical experience, ideally in internal medicine, cardiac, family, concierge, executive health, or other high performance care
- Passionate about high-touch, patient-centered care that focuses on prevention, longevity, and performance optimization
- Humble with high relational intelligence
- Organized, detail-oriented, and proactive
- Adept and conversant with various software systems (EPIC, Google Workspace, Microsoft Office); interested in leveraging digital tools to improve care
- Strong communicator and team collaborator
- Committed to excellence in all areas of service delivery
- Adaptable, proactive, curious, and energized by working in a dynamic, fast-growing company
- Comfortable balancing clinical care and operational workflows
- Excited to practice within an innovative primary care model
Key Responsibilities
- Clinical Leverage & Support
- Serve as a trusted clinical extension of Dr. Levy and Dr. Hollett, who deliver a high-level of care and engagement through clear, professional, and timely responses to texts, emails, and calls for simple medical inquiries.
- Triage messages, assess diagnostics, respond to acute issues, manage labs, and ensure appropriate escalation.
- Serve as a visible, accessible point of contact for SHIFT members, enhancing the experience of next-level concierge care.
- Continuity & Documentation
- Maintain organized and current EHR documentation (EPIC).
- Coordinate closely with Clinical Care Coordinators (CCCs) to ensure seamless communication and care tracking.
- Field and address prescription refill requests efficiently and accurately.
- Coordinate referrals and specialty appointments.
- Member Engagement
- Proactively manage relationships with SHIFT Members to ensure clarity, forward motion, and exceptional service.
- Anticipate and address emerging concerns; embody the SHIFT clinical philosophy in all interactions.
- Systems Integration & Innovation
- Collaborate with subject matter experts in various care disciplines to support delivery of holistic care plans.
- Participate in refining protocols and internal workflows; help build the system while operating within it.
Qualifications
- Board-certified Nurse Practitioner or PA.
- Licensed to practice in the State of Illinois.
- Minimum of 3 years of clinical experience preferred, ideally in primary care, internal medicine, or concierge medicine settings.
- Demonstrated ability to coordinate complex care and communicate effectively across teams.
- Strong organizational skills and a proven track record of clinical responsiveness.
- Prior experience in high-service, high-accountability environments preferred.
- EPIC EMR experience a plus.
